Clients will receive the service rate sheet prior to their first scheduled service so they can make informed decisions and understand the cost of care.

Services are non-clinical and do not replace therapy, psychiatric care, medical treatment, or counseling provided by licensed mental health professionals.

No clinical diagnosis or treatment plan will be created.

                                               IMPORTANT DISCLOSURES

• If a client’s needs require licensed mental health treatment, God’s Chosen LLC will provide appropriate referrals.
• Participation is voluntary, and clients may discontinue services at any time.
• Confidentiality is maintained to the fullest extent permitted by non-clinical service standards and by law.

                                              ACKNOWLEDGEMENT

Clients will be asked to review and sign an acknowledgment form confirming they understand:

• The non-clinical nature of the services
• The difference between Christian counseling and licensed therapy
• The service rates and payment expectations

By continuing, I acknowledge that the services provided by God’s Chosen LLC are non-clinical and are not a substitute for licensed mental health treatment. I understand that I will not receive a diagnosis, treatment plan, clinical notes, or therapy, and I agree to seek a licensed provider if advised or if needed.

                                   CHRISTIAN COUNSELING DISCLOSURE

God’s Chosen LLC provides faith-based, non-clinical counseling and spiritual support rooted in Biblical principles. The services offered:

• Do not include licensed mental health therapy
• Do not include diagnosis or clinical treatment
• Are not a replacement** for professional psychological, medical, or psychiatric care
• Are intended solely as spiritual guidance and supportive services

Clients are encouraged to seek a licensed therapist, psychiatrist, or medical professional for mental health conditions, diagnoses, or treatment needs. God’s Chosen LLC may offer referrals when appropriate.

By engaging in services, clients acknowledge understanding the non-clinical nature of support.
